Insurance:

This information may be entered via the HEALOW patient portal, emails in .pdf legible formats, or updated in person in the office. Please log in to your HEALOW patient portal to add this information. We accept many forms of insurance including but not limited to:

Medicare and many managed care replacements and plans, Worker's Compensation with adjuster approval, OWCP DOL cases, auto Med Pay with a valid AOB assignment of benefits on file, most major insurance plans including Anthem/BCBS, Aetna, some Tricare plans, some UHC plans, some Cigna plans. We are now providers for Curative. We have joined Humana effective 10/1/2026. If your insurance requires a referral or a prior approval, it is the patient/guardian's responsiblity to make sure those are in place prior to treatment. Please note that this office does not accept nor recognize down-coding.
